Is blood in sperm a sign of cancer?

Usually not. Blood in semen (hematospermia) is most often a harmless burst small blood vessel, a recent prostate exam, inflammation or infection, and often clears on its own. Cancer is an uncommon cause. See a doctor promptly if it persists or recurs, comes with pain, blood in urine or a testicular lump, or you are over about 40.
